Pre-Lesson Forms
When you schedule a private class, you will be requested to fill out a Physical Activity Readiness Questionnaire. This questionnaire is designed to inform us of important information regarding your health and any medical requirements you may have that may impact your suitability for participation in pole fitness, or that may require consideration during lessons. The data collected in this questionnaire includes: first name, last name, and an emergency contact name and phone number. Additionally, sensitive data is collected, including information about health status, medical conditions and disabilities. This data is kept confidential, in a password-protected account, and is not shared with third-parties, unless absolutely necessary in the event of an emergency. If applicable, after 12 months the data will be erased and a new Physical Activity Readiness Questionnaire will be given to you.
Online classes
Online private classes are recorded for insurance purposes (after consent has been given). These recordings are stored confidentially and as securely as possible by us, for a period of 30 days, provided the class proceeds without any incidents, falls, or injuries. After this period, the recordings are permanently deleted. In the event of any such incidents, the recordings are retained for a minimum of 24 months from the date of recording before deletion.
